      Fall 2003, Biology401: Assignment #7 - Institutional Animal Care and Use Committee (IACUC) Forms.

      Task: In this assignment, you will download a pair of WORD forms, fill them out, and send them as attachment files to Itzick in order to obtain permission from IACUC to conduct your research with animals this semester.

      Assessment:

      • +20 points if completed by Tuesday 14 October 2003.

      Detailed Instructions:

      1. Download the two MS-WORD files posted below and save them to your disk.
      2. Open each in a word word processor (such as MS-Word) and enter your information directly on each page.
      3. If you have any questions, or are uncertain what to write, either find us and ask, or send an email with (1) the form name and question text, (2) a draft of the answer you have come up with to date, and (3) description of your uncertainty.
      4. Spell check and proof read your answers.
      5. IMPORTANT: Save your file under a different name such as IACUC_YourLastNames.doc
      6. Copy the following text to the email subject line: Biology401: Assignment 7 - IACUC forms
      7. Send your completed forms AS ATTACHMENTS TO Itzick Vatnick at vatnick@pop1.science.widener.edu and cc: it to yourself and to yuor other group members. Please do NOT send these files to BWG.
      8. Please submit only one set of forms per group.
